Stop Smoking Services Quarterly Monitoring Return 2008-09
The NHS Stop Smoking Services Quarterly Monitoring return is made available to Primary Care Trusts (PCTs) using a web based tool at the start of each year. PCTs then submit completed returns to the Information Centre on a quarterly basis.
The 2008-09 return form remains in a similar format to that used in previous collection years. It contains 5 worksheets; four worksheets, one for each quarter, in which to input the required data and a worksheet titled "Summary" which combines the results of the quarterly worksheets.
When completing data for a quarter, previous quarters' data can be updated during that particular collection year.
The return form has built in validations and any errors are flagged to the user. PCTs are asked to address these errors before uploading their spreadsheet.

The 2008/09 return form is available for download below. Please note we are making the form available on this website for the information purposes of a wider audience, PCTs should continue to obtain the form using the web based tool.
