Information for intermediate care
Intermediate care offers a way of helping to move people out of acute hospitals as soon as their health needs justify that. It can also offer a way to deliver the intensive packages of care that might prevent the need for hospital admission in the first place.
The Adult Care Information Network is looking at how to record information on intermediate care. This was the focus of the last two ACIN meetings, in Belfast on 31 January and London on 27 June 2007. Presentations and other resources on intermediate care and presentations can be found on the pages from both those meetings.
