Passing on vital medical information
The NHS Adoption Service can pass vital medical information between the GPs or Consultants of Adoptees and Birth Relatives. This service can only be offered if you need to pass on information relating to a hereditary medical condition.
If the person sought is still recorded as alive, is currently registered with a GP in England or Wales, and can be correctly identified by the NHS Adoption Service, medical information will be forwarded. The NHS Adoption Service does not facilitate any contact between the applicant and the person sought and will pass on no other information.
How do I apply?
You should write to us and supply:
- all known details pertaining to your birth parents or adopted child. (A copy of your original birth certificate or adoption papers would be useful if available).
- a letter from your GP or consultant explaining your medical condition (the letter must not quote any of your personal identifiable details ie, name, date of birth)
Since information given to medical practitioners is confidential, this service provides the only means of passing on medical details and is subject to the consent of both doctors involved.
