Alive or deceased service

To advise if a birth parent or adoptee is recorded as being alive or deceased you must apply via one of the organisations listed below.

If you want to use this service, the organisation acting on your behalf would need to provide the NHS Adoption team with all known details pertaining your birth parents/adopted child (ie: names, addresses, dates of birth if known and a copy of the original birth certificate or adoption papers). These assist us to conduct a more thorough search of our records.

Where it can be established that the person sought has died, the NHS Adoption team will advise how to obtain a copy of the death registration details provided that person has died in the name supplied by the applicant.

This service is only available through one of the following organisations who should quote their Ofsted, Charity or Registration Number on any application:

  • social services
  • local councils
  • registered charities
  • registered adoption agencies
  • registered adoption support services
