You will need to provide us with a study protocol which gives an overall description of the proposed research study. This forms part of the ethical approval from the National Research Ethics Service (NRES).

The protocol needs to describe:

• the proposed involvement of The NHS Information Centre
• use of the Medical Research Information Service for long-term follow-up, current status checks or list cleaning
• plans for data extracts from The NHS Information Centre
