Patient consent forms
If you are requesting patient identifiable data from the MRIS, you will need to get informed consent from all those people. As part of your application you should send us a copy of the form you intend to use.
To gain adequate informed consent The NHS IC recommends the following wording:
'I understand that information held and managed by The Health and Social Care Information Centre and other central UK NHS bodies may be used in order to help contact me or provide information about my health status.'
If the study subjects agree to this it provides consent for flagging or tracing for death/cancer and consent for providing PCT details in the future.
If you want us to process information without adequate informed consent, you will need support under Section 251 of the NHS Act 2006, instead.
