Please follow the application checklist below, to make sure that you have all the correct supporting documentation and approvals before submitting your application form.

Application Checklist
What we need from you:
For your application we will need:
- MRIS Application Form (PDF 262KB)
- Study Protocol
- Ethical Approval
- Commissioning letter or evidence of funding
- Patient Consent Forms or S251 letter of approval
- Patient Information Leaflet(s) you will use when gaining consent
- PCT, GP and Patient Letters (if applicable)

To process your application we need to check:
- your supporting documentation
- whether you require data from the Medical Research Information Service
- whether you require cancer registrations
- whether you have adequate informed consent
- whether support is required under Section 251 of the NHS Act 2006.
This is needed if you want to use patient identifiable information without the person's consent and was formerly provided by section 60 of the Health and Social Care Act 2001 re enacted under S251 of the NHS Act 2006.
